- Eye exam $10 deductible every 12 months
- Eyeglass lenses $20 deductible every 24 months
- Frames $130 retail allowance and a 20% discount on any price over the allowance
- Contacts, instead of frames and lenses, have a $20 deductible every 24 months and a $130 retail allowance
- 40% off additional eyeglasses

- First, run a quote to view dental plans
- Select your dental plan and choose to enroll
- A popup vision rider will appear and select “Add Vision” if it's available in your state
